The task of Home Health is to keep a patient in their home as long as possible and also to keep them from illness or injury. Some of the ways our nurses are able to do this is with medication set ups and medication review, assessing the patient’s condition during every visit and let their primary doctor know if there are any concerns, check vitals for the patient and also provide peace of mind for the patient’s family knowing that a nurse is checking on their family member.
